4. Valid Government ID’s

B. OFW Major Documents

1. COEC

2. COE – Contract of Employment photocopy

3. Passport with Entries

4. Payslips (Latest 3 months)

5. Valid Visa

6. If Seafarer , needs Seaman’s Book with entries

C. Self Employed Major Documents

1. ITR (last 2 years )

2. FS -Financial Statements (last 2 years)

3. Business Registration , Mayor’s Permit

4. SEC Registration (if Corporation)

5. Bank Statements for Last 6 months

6. List of Suppliers and Clients w/ Contact Nos.

7. Vehicle OR/ CR if transport Business

8. Other Major Documents

9. Proof of billing

10. Two valid identification card (ID)( for PB, CB & AIF)

11. Birth Certificate or Marriage Certificate

12. Notarized / Consularized SPA (if OFW)

13. Post dated checks for downpayment and amortization (1 year)

14. Oath of Allegiance ( if dual Citizen )

15. Certificate of Non Marriage ( if with common law partner and with kids )
